Euglycemic diabetic ketoacidosis by sodium glucose co-transporter inhibitors: Real but preventable concern

چکیده
We are thankful for the comments on ‘Sodium glucose co-transporter (SGLT2) inhibitor: Patient safety and clinical importance’, and we appreciate the concern raised for its risk of euglycemic diabetic ketoacidosis (DKA). During clinical development programs for a new drug approval, it is not uncommon that some unexpected safety issues may go unobserved. Interestingly, the incidence of ketoacidosis for three SGLT2 inhibitors empagliflozin, dapagliflozin and canagliflozin were 0.1%, 0.1% and 0.5%–0.8% in EMPA-REG Outcome trial, DECLARE trial and CANVAS trial, respectively. This is suggestive of acceptably very low occurrence.[1]

Safety of sodium-glucose co-transporter 2 inhibitors during Ramadan fasting: Evidence, perceptions and guidelines
Sodium-glucose co-transporter 2 (SGLT2) inhibitors are a new glucose-lowering therapy for T2DM with documented benefits on blood glucose, hypertension, weight reduction and long term cardiovascular benefit. They have an inherent osmotic diuretic effect and lead to some volume loss and possible dehydration. There is some concern about the safety of using SGLT2 inhibitors in Muslim type 2 diabete...
متن کاملEuglycemic Diabetic Ketoacidosis With Prolonged Glucosuria Associated With the Sodium-Glucose Cotransporter-2 Canagliflozin
Sodium-glucose cotransporter-2 (SGLT2) inhibitors improve glycemic control by a reversible inhibition of the sodium-glucose cotransporters in the renal proximal tubules resulting in increased urinary glucose. This unique mechanism, independent of insulin secretion and beta cell function, has made this class of medication desirable in patients with type 2 diabetes. However in May 2015, the US Fo...

Sodium glucose co-transporter (SGLT-2) inhibitor is a relatively new medication used to treat diabetes. At present, the Food and Drug Administration (FDA) has only approved three medications (canagliflozin, dapagliflozin and empagliflozin) in this drug class for the management of Type 2 diabetes. In May 2015, the FDA issued a warning of ketoacidosis with use of this drug class. Risk factors for...
متن کاملMetabolic ketoacidosis with normal blood glucose: A rare complication of sodium–glucose cotransporter 2 inhibitors
Ketoacidosis is a significant and often a life-threatening complication of diabetes mellitus seen mostly in type 1 diabetes mellitus as well as occasionally in type 2 diabetes mellitus. Diabetic ketoacidosis usually manifests with high blood glucose more than 250 mg/dL, but euglycemic diabetic ketoacidosis is defined as ketoacidosis associated with blood glucose level less than 250 mg/dL. Norma...

Since the approval of sodium-glucose cotransporter 2 (SGLT2) inhibitors by the US Food and Drug Administration for type 2 diabetes, there have been several reports of euglycemic diabetic ketoacidosis in patients using this class of medication. We present a case of euglycemic diabetic ketoacidosis where ketonemia and glucosuria persisted well beyond the expected effect of dapagliflozin. Our pati...
